650.4 Hospitals or sanatoria (annual reports). (a) Hospitals and sanatoria shall report annually on form H-1 consisting of three sections as follows:

(1) general information;

(2) service statistics--inpatient; and

(3) service statistics--outpatients (excluding dispensary).

(b) Hospitals and sanatoria which are in receipt of public moneys shall also report annually financial data on report form H-2 consisting of three sections as follows:

(1) current income;

(2) current expenses; and

(3) balance sheet as of last day of year.
